Transport Intramedullary Nail for Nonunion.

Nonunions with bone defects create technical challenges for fracture healing. All-internal bone transport with an intramedullary nail offers benefits over the other bone segment transport options including being simpler for the patients, avoiding the associated soft tissue issues that externally based implants or techniques and minimizing residual deformity by maintaining the anatomical axis along the intramedullary nail. This article reviews preoperative planning, technical tricks and specific techniques for internal bone transport, including the corticotomy, distraction, docking site and regenerate management, and the available literature.

Intramedullary Bone Transport Nail for the Treatment of Segmental Tibial Bone Defect After Acute Traumatic Bone Loss.

Medullary bone transport is a valuable technique to perform distraction osteogenesis with all-internal components. This technique has notable advantages over other techniques which rely on external pins, wires, and frames. Preoperative templating and surgical planning are critical to the success of this procedure to ensure that movement of the segment across the defect can be performed based on the specifications of the nail. The case of a young patient with a 67 mm diaphyseal tibial defect after an open tibial shaft fracture who had successful reconstruction with a medullary bone transport nail is presented.

Bone defects are among the most challenging problems treated by limb reconstruction surgeons and orthopaedic traumatologists. Bone transport is a biological reconstruction technique which harnesses distraction osteogenesis to create new bone in critical bone defect. This technique has been successfully performed with a variety of internal and external solutions. This work introduces a newly available technology which provides a fully implantable, all-internal solution (eg, without the need for external fixation) for bone transport with the option to simultaneously address residual limb length discrepancy.

Robust graphene oxide composite aerogel for micro solid-phase extraction of methorphan enantiomers from biological matrices.

Methorphan exists as two optically active enantiomers, levomethorphan (LVM) and dextromethorphan (DXM). DXM is a widely utilized central antitussive, whereas LVM is a strictly regulated controlled substance. This study established a method combining micro-solid-phase extraction with enantioselective high-performance liquid chromatography-mass spectrometry (LC-MS) for the simultaneous determination of LVM and DXM in human urine and nail samples. A spherical porous graphene oxide composite aerogel (GO@CNF/CMC) served as the extraction sorbent. It was synthesized by covalently crosslinking graphene oxide (GO), cellulose nanofibers (CNF), and carboxymethyl chitosan (CMC) via a combined sol-gel and dropwise addition strategy. Aerogels with various GO mass fractions were fabricated for sorption optimization. With a maximum GO loading of 11.2 wt%, the aerogel delivered superior extraction efficiency owing to its large specific surface area and synergistic intermolecular interactions including hydrogen bonding, π-π stacking and electrostatic attraction. The aerogel exhibited excellent mechanical stability and recyclability, with nearly consistent extraction efficiency over 15 extraction cycles. Under optimized extraction conditions, the method exhibited good linearity in the range of 0.05-10 μg/L. The limits of detection were 0.01 μg/L for both LVM and DXM. The spiked recoveries in urine and nail samples ranged from 81.6% to 108.4%, and matrix effects ranged from 83.3% to 105.7%. This method provides a facile, rapid, green and sensitive analytical strategy for the trace analysis of methorphan enantiomers in complex biological matrices.

Onychomycosis: clinical and dermoscopic characteristics, diagnosis, and therapeutic management.

Onychomycosis is a chronic and recurrent nail infection whose worldwide prevalence continues to increase, affecting 5%-10% of the general population and up to 50% of individuals older than 70 years. Despite the availability of multiple effective therapeutic options, current management practices remain suboptimal, and recurrence rates continue to be high. Recent evidence points to persistent shortcomings in clinical practice, including the underuse of confirmatory diagnostic tests, which entails a greater risk of diagnostic errors, and the frequent initiation of empirical treatment without prior etiological verification, which is associated with treatment failure and resistance, lack of adherence, and persistence of infection. Overall, onychomycosis remains a prevalent and complex clinical problem whose control requires greater diagnostic accuracy, rational use of antifungal agents, and standardized strategies for follow-up and prevention of recurrence.
